PSYCHIATRIC ILLNESS DURING PREGNANCY

Association between hyperemesis gravidarum and depression: a national register-based controlled study in Finland. Terävä-Utti E, Nurmi M, Laitinen L, Rissanen T, Järvenpää T, Polo-Kantola P. Lancet Psychiatry. 2025 Nov;12(11):841-851.

Women with hyperemesis gravidarum were more likely to have pre-pregnancy and new onset depression compared to controls, with earlier onset after delivery and persistent risk for years postpartum.

Birthing people with a history of concussion had increased risk of severe maternal mental illness, especially those without prior mental illness.

Safety of Valproic Acid Use in Pregnant Women: A Systematic Review and Meta-Analysis. Meta-analysis reveals significantly increased risks with prenatal valproate exposure for neural tube defects, congenital heart defects, cleft lip/palate, genitourinary, and musculoskeletal anomalies compared with other AEDs.

Intermediate or poor CYP2C19 metabolizers may require dose adjustment for escitalopram in pregnancy, as concentrations may become subtherapeutic.

A childbirth/parenting booklet alone may not reduce paternal PPD risk; more interactive or tailored interventions may be needed.

Training fathers during pregnancy in coping strategies reduced postnatal depression scores in both mothers and fathers.

Psychotropic Medication Exposure via Breast Milk: A Population-Based Descriptive Study in Denmark. Among 446,573 Danish infants exclusively breastfed, 17.6 per 1000 were exposed to at least one psychotropic medication via breast milk.

Patients reported high treatment satisfaction with buprenorphine during the extended postpartum period, but most had a goal of discontinuation; social support for long-term treatment was limited.

Cost-Effectiveness of Treatment for Opioid Use Disorder in Pregnancy and Its Impact on Birth Outcomes. Both methadone and buprenorphine were viable for OUD management in pregnancy, but buprenorphine offered greater lifetime benefits for infant outcomes.
